Data Protection Privacy Statement Of Arfon Physiotherapy Ltd.

Your consultations with us are treated as confidential as far as is permitted by the General Data Protection Regulation (GDPR). We are registered with the Information Commissioner’s Office as a Data Controller.

As part of our treatment of you, we are required by law to make a record of your care with us. You have a right to see these records if you wish – a fee of £10 can sometimes be applicable and the records will be provided within a month.

We are required to store your records for a legally determined period after your treatment with us has finished. Generally, adult records will be stored for 8 years, and children’s (persons under 18 years of age) records will be stored until they are 26 years old. Records may be stored for longer for other patient groups. Once the retention period has expired, the records will be securely and permanently destroyed.

We may need to share some of the information you give us, or that we find through examination, with other health professionals in order to give you the best-available advice and treatment. We do not need your written permission to do this, but we will always seek to ensure that you are aware of the communication we have with other health professionals. You have the right to prevent us sharing information. If you expressly forbid us from sharing information with other health professionals directly involved in your care, then it may prevent us from giving you the most appropriate care, and in extreme cases may mean we cannot treat you.

We do not share your information with third parties (such as solicitors or employers) without obtaining your written permission beforehand. We will tell you who is asking for the information, what they are asking for, and what they need the information for. You can choose whether you give us written permission. If you do not give your permission then it may prevent us from helping others to assist you, and in extreme cases may mean we cannot treat you.

If your treatment is being paid for under a contract of insurance the terms of that insurance contract may require us to tell your insurer:

  • Your name, address, date of birth and insurance policy number
  • Any pre-authorisation number you may have for treatment with us
  • The dates you attend, or fail to attend, for assessment and treatment with us
  • A short description of the condition we are treating you for, which may include diagnosis and relevant history.
  • The name of the person who has referred you to us
  • Our charges to the insurance company.

If you do not allow us to pass this information about you to your insurer as part of their monitoring of their insurance contract with you, then we may not be able to treat you as an insurance-funded patient. You should discuss any concerns with your insurance company prior to commencing treatment with us.

There are certain limited circumstances in which the law requires us to share information with others, even if you tell us not to. For example, to insurance companies, loss assessors, regulatory authorities and other fraud prevention agencies for the purposes of fraud prevention and to comply with any legal regulatory issues and disclosures. Also, to any legal or crime prevention agencies and/or to satisfy any regulatory request (including recognised practitioner bodies) if we have a duty to do so or the law allows up to do so.

Any information you choose to give us will not be used for marketing purposes by us.

In order to provide the highest level of customer service possible, we need to keep accurate personal data about you. You can help by informing us of any changes such as change of address/telephone number when they occur.
